Chart Notes

Learn how to use the tools within Chart Notes to create quick and effective records for your patients.

This guide provides step-by-step instructions on how to create and manage chart notes effectively using Suno. Below, you'll find detailed information on how to utilize Suno’s features to enhance your chart note-taking process.

Types of Chart Notes

There are three different types of notes that you can find in the Chart Notes section:

  • Patient Visit Note: Generated automatically when a patient has been marked as arrived for their appointment. Under Type you will see the appointment type the chart note is attached to:

  • General Chart Note: Created when you select the New Chart Note button; these will not be tied to any appointments.

  • Billing Note: Created when selecting the New Billing Note button. Useful for keeping track of any billing information that you would like to retain in their Chart Notes section.

Creating a New Chart Note

  1. Automatic Chart Note Generation:

    • When a patient is marked as arrived for their appointment, Suno automatically generates a new chart note tied to the appointment. This is called a Patient Visit chart note.

  2. Manual Chart Note Creation:

    • If a separate chart note is needed, click the New Chart Note button located at the top-right of the chart notes section.

    • Note: Manually created notes are labeled as General Chart Note.

  3. Draft Status:

    • New chart notes are initially in Draft Status and will appear red in the chart note header. Clicking into the note opens a word processor with editing tools similar to Microsoft Word or Google Docs.

Editing Chart Notes

  • Utilize features such speech-to-text, font styling, and adding lists, links, images, and tables.

  • Templates can be added for standardizing repetitive information.

Adding ICD-10 Codes

  • For notes generated from appointments, there is an option to add ICD-10 codes.

  • Search and select the necessary ICD-10 codes for inclusion.

  • Use Add Most Recent Codes to import codes from the last chart note for consistency.

Utilizing AI in Chart Notes

  • AI can enhance and organize your chart notes.

  • Write a brief description, then select Enhance Note to expand it using AI.

  • The AI can format notes in the SOAP format and add ICD-10 codes based on your input.

  • Note: Please be sure to review AI-enhanced notes for accuracy.

Saving and Completing Chart Notes

  • Suno auto-saves your work every few seconds, indicated by a spinning arrows icon:

  • When the chart note is saved, you will then see a green cloud icon appear indicating that the current notes are saved:

  • To mark a chart note as complete, select the “Complete” option. The header will turn gray, and the appointment checkbox will automatically check if the note is completed.

Tagging Chart Notes

As with many areas within Suno, you can create and utilize Tags to help color-code and organize your notes (if you'd like a refresher on how to create tags, you can follow this documentation here). Once you have created the necessary tags, you can click the + Add Tag option beneath the chart note header as shown below:

These tags will also be visible when the chart note is closed for quick identification:

Filtering and Searching Chart Notes

  • Filter by Type: General Note, Patient Visit, or Billing Note.

  • Filter by Status: Draft or Completed.

  • Filter by Chart Note Tag: Filter by the tags that have been added to your chart notes.

  • Search: Use the search bar to find specific text within notes.

  • Expand/Collapse: Options available for viewing preferences.
