Patient Portal

Learn how to access the Patient Portal in order to fill out Onboarding forms in-office with your patients

This guide outlines the steps to assist a patient in completing onboarding forms using the Patient Portal when they are unable to do so independently.

You can access the Patient Portal using a laptop, desktop, or tablet.

First Step: Send Onboarding Forms

  1. Access the Onboarding Section:

    • Locate the patient’s profile by searching for their name or Suno ID.

    • Once in the patient’s profile, navigate to the Onboarding section.

  2. Send New Onboarding Forms:

    • While typically done during the Scheduling phase, if you need to send new or additional onboarding forms you can click the + New Form Submission Request button in the top-right corner.

    • Select the required forms.

    • Choose the preferred communication method (e.g., email or text).

    • Optionally attach the forms to an appointment.

    • Click Send Request to deliver the forms to the patient.

Second Step: Access Patient Portal

  1. Access the Patient Portal:

    • Click the Patient Portal button within the patient’s profile.

    • A new tab will open with the Patient Portal login screen.

  2. Log In to the Patient Portal:

    • Enter the patient’s associated email address or phone number.

    • Use the patient’s Date of Birth as the password.

    • Click Sign In after verifying the information.

  3. Complete Onboarding Forms:

    • In the Patient Portal, go to the Pending Forms section.

    • Open each form that needs to be completed.

    • Submit the forms once finished.

    • All submitted forms will automatically be added to the patient’s profile in Suno.
