This guide outlines the steps to assist a patient in completing onboarding forms using the Patient Portal when they are unable to do so independently.
You can access the Patient Portal using a laptop, desktop, or tablet.
First Step: Send Onboarding Forms
Access the Onboarding Section:
Locate the patient’s profile by searching for their name or
Suno ID.Once in the patient’s profile, navigate to the Onboarding section.
Send New Onboarding Forms:
While typically done during the
Schedulingphase, if you need to send new or additional onboarding forms you can click the+ New Form Submission Requestbutton in the top-right corner.
Select the required forms.
Choose the preferred communication method (e.g., email or text).
Optionally attach the forms to an appointment.
Click Send Request to deliver the forms to the patient.
